Clear lungs, a dying RV

POD 3 after a hemicolectomy. She stood up, clutched her chest, and desatted. Lungs are clear. RA 22, PA 52/32, PCWP 10, CO 2.2, PAPi 0.91. The RV is a balloon on the bedside echo you have not officially ordered. Fluids are running because ‘obstruction needs preload.’ That RV is already drowning.
Untreated tamponade, massive PE, or a crashed CPO/PAPi will keep dropping MAP and climbing lactate. Name the tracing, then move.
