Six months of unexplained dyspnea. Echo guessed at PH. You are at the table: PA 78/32, mean 48, PCWP 11, CO 3.8. PVR is just under 10 Wood units. This is pre-capillary physiology. The referring team has already written for a nitroprusside trial ‘to unload.’ That is the wrong ventricle.
Untreated tamponade, massive PE, or a crashed CPO/PAPi will keep dropping MAP and climbing lactate. Name the tracing, then move.
