Inferoposterior infarct, new holosystolic murmur, flash pulmonary edema. The digital wedge says 22. The paper tracing says v waves to 44. Acute MR until the echo disagrees. A pure pressor will make the leak worse. Afterload reduction — if the MAP allows — is the catheter’s advice.

Untreated tamponade, massive PE, or a crashed CPO/PAPi will keep dropping MAP and climbing lactate. Name the tracing, then move.
