The fellow is proud of the numbers: RA −6, PCWP 0, PA 18/2, and the patient is sitting there discussing lunch. MAP is 88. The transducer is hanging off the bed rail, closer to the floor than to the 4th intercostal space. Re-zero before you treat a fairy tale.

Untreated tamponade, massive PE, or a crashed CPO/PAPi will keep dropping MAP and climbing lactate. Name the tracing, then move.
