You are called to a freshly cannulated 52-year-old with influenza ARDS, 20 minutes off the table. Fem-jug VV is online but the sats never climbed and the last gas is still a respiratory acidosis. Console: RPM 2500, flow 2.8 L, FdO₂ 0.6, sweep 2 L. The cannulation team has left. Stabilize gas exchange.
Uncorrected hypoxemia, shock, or a dead circuit progresses to pre-arrest, then a code.
