VV-ECMO. Monitor SpO₂ is 92%, but the fellow just hung an ABG: pH 7.10, PaCO₂ 78. Flow 4.0 L, FdO₂ 0.8, sweep still 1.5 L from cannulation. The patient is tachypneic on low vent settings. The RT is asking which knob to turn.
Uncorrected hypoxemia, shock, or a dead circuit progresses to pre-arrest, then a code.
