VV-ECMO, sats look fine on the monitor (92%) but the fellow just hung an ABG: pH 7.10, PaCO₂ 78. Flow 4.0 L, FdO₂ 0.8, sweep still 1.5 L from cannulation. The patient is tachypneic on low vent settings and the RT is asking which knob to turn.
Uncorrected hypoxemia, shock, or a dead circuit progresses to pre-arrest, then a code.
