VA-ECMO. The air-detect alarm just screamed and flow collapsed. MAP is falling through the 50s and the patient looks peri-arrest. You can see foam in the tubing. You have seconds, not a workup.
Uncorrected hypoxemia, shock, or a dead circuit progresses to pre-arrest, then a code.
