31-Fr Avalon. Morning echo: jet on the TV, bicaval seat acceptable. Overnight someone chased a sat of 92% up to RPM 4300. Flow 4.7 L, pre-oxy SvO₂ 86%, Pven −78, pfHb 28, circuit starting to chatter. Patient sat is 84%. Dual-lumen recirc and shear climb when you spin past the anatomy.
Uncorrected hypoxemia, shock, or a dead circuit progresses to pre-arrest, then a code.
