Case 13 — Rising pressures, oliguria

Handover

Massive resuscitation for pancreatitis. The abdomen is tight. PIP and Pplat are both 38, urine 5 mL/h, MAP 56 on rising pressors. Bladder pressure has not been measured. The fellow wants more PEEP for “stiff lungs.”

Uncorrected hypoxemia, auto-PEEP shock, tension, or abdominal compartment progresses to pre-arrest, then a fail. Read the scalars before you turn a knob.

Goals

  • · IAP measured
  • · Abdomen decompressed
  • · IAP ≤ 15
  • · Pplat ≤ 28
