Anterior STEMI, shock in the lab, Impella CP just placed from the right femoral. They left it at P-2 “to confirm position.” Placement signal looks aortic. MAP 54 on a modest pressor, lactate 5.2, CPO 0.42, urine a trickle. The implanting team has gone to the next case.
Uncorrected shock, suction, or a misplaced inlet progresses to pre-arrest, then a fail.
