Handover
They left the norepinephrine running after implant. SVR is 2100, MAP 92, Impella CP at P-8 — and flow is stuck at 1.8 L. Lactate is 3.4. Placement signal is a clean aortic tracing. The pump is pushing into a brick wall. Afterload is the clamp.
Uncorrected shock, suction, or a misplaced inlet progresses to pre-arrest, then a fail.
