Inferior STEMI, isolated RV infarct, Impella RP parked at P-3. CVP 22, MAP 56, PAPi 0.7, liver edge down, urine off. The RA is a lake and you are not forwarding it into the PA. Titrate the RP. The LV cannot eject what it never receives.
Uncorrected shock, suction, or a misplaced inlet progresses to pre-arrest, then a fail.
